Skilled Nursing Facilities benefit from a 100% share of savings based on lower expenses from improved outcomes and fewer unnecessary hospitalizations.
Residents enjoy supplemental benefits not offered by traditional Fee for Service Medicare including dental care, podiatry, hearing and much more.
ACO REACH for Providers
Provider Partners Connect Care ACO REACH program was founded by a physician for physicians to deliver high-quality, coordinated care for Medicare Fee-for-Service Beneficiaries.
With a focus on high-needs patients, we help clinicians improve outcomes and simplify value-based care. Primary Care Physicians benefit from a payment program that includes a premium above the Medicare Fee Schedule for E&M codes, along with annual incentive payments based on shared savings tied to improved quality and reduced hospitalizations.
